UCH Ibadan School of Health Information Management (SHIM) HND Admission Form for 2020/2021 Academic Session

Applications are invited from suitably qualified candidates for admission into the University College Hospital (UCH), Ibadan, Two-Year Higher National Diploma (HND) Programme in Health Information Management for the 2020/2021 academic session.  The course, which is non-residential, shall commence in October 2020.

Conditions for Admission into UCH Ibadan SHIM HND Programme.

Selection of candidates will be on the basis of good academic qualification, good performance in the computer base test (BT), and oral interview as well as presentation of relevant ORIGINAL CERTIFICATES.

Note:MERIT AND PASS IN (NATIONAL DIPLOMA) ND SHOULD NOT APPLY EXCEPT WITH ND PRACTICING LICENSE OR 3 YEARS WORKING EXPERIENCE.

The Computer Base Test (CBT) for HND will be in:

• English Language
• Health Records Management
• Anatomy & Physiology

Please note that:

• pregnancy is not allowed during training
• selected candidates will be required to undergo and pass medical examination, first thing after admission

How to Apply for UCH Ibadan SHIM HND Form.

Interested HND applicants are to pay N7,500 for Application and Screening fee respectively into the TSA ACCOUNT (TSA/UCH/SHIM/ADMISSION FORM) in any commercial bank.  Applicants are expected to convert their bank teller into a receipt at the Cash Office of the Finance & Accounts Department, University College Hospital, Ibadan.  The photocopy of the receipt of payment and the photocopy of the bank teller should be forwarded/brought to the Administrative Office of the School.

All applicants must have a functional E-mail address and are expected to collect their access code from the school Administrative Office to apply online, that is, to Log onto: portal.uch-ibadan.org.ng.  Application must be completed online.

The screening exercise for the ND programme and HND entrance examination will be communicated to the candidates via their e-mail address.

Successful applicants will be invited for an oral interview at a later date through text messages and the School’s website.
