CHAPTER ONE

BACK GROUND OF THE STUDY

Introduction

Vesico Vaginal Fistula is a preventable disease that is more prevalent among the less privileged and marginalized member of the population; the poor, young, illiterate girls and women in the remote rural areas of the world, where access to emergency obstetric care, family planning services and skilled birth attendance are unavailable poorly utilized. Olatrawo (2010)

Vesico Vaginal Fistula is an abnormal opening in the urinary bladder and vagina. Vesico Vaginal Fistula appear to have been in existence since antiquity evident by reference made to genital fistula in Ebers Papyrus and in an Egyptian mummy in 2000BC and respectively. Olatrawo Β (2010).

Research has shown that VVF patients are usually young girls who have no formal education, no means of live hood, and given out in marriage by their poverty stricken parents to poverty stricken husbands. As a result, patient like that are normally malnourished in pregnancy and no personal income to take care of them and to attend antenatal clinic. OlatrawoΒ  (2010).

Patient with VVF comes almost exclusively from poor families and communities.”Communities can be poor if they lack basic facilities to take care of their members. The report describes the women as malnourished from birth, susceptible to diseases, chronically anaemic and physically stunted.

Poverty leads to malnourishment, and small height. It leads to not having money to travel the distance to attend antenatal care, and invariably lead to obstructed labour. WHO (2005).

Obstetric Vesico Vaginal Fistula have from the industrialized World, despite the efforts of many charitable organizations, they continue to occur in epidemic numbers in developing countries. The national and Local Government of these countries do not have either the resources or the political will to address this problem and help these outcase women. WHO (2005).

The number of vesico vaginal fistula in a region reflects the quality and the level of prenatal antenatal, intra natal and post natal care delivered by the local health systems. In region where health care (particularly maternal health care) is poor or absent, the number of obstetric fistulas is likely to be high.

Based on the above context the study is designed to find out the prevalence of Vesico Vaginal Fistula As seen in Maryam Abacha Women and Children Hospital Sokoto from January 2015 to December 2016.

The prevalence of vesico Vaginal fistula seem to be high in area where teenage pregnancy, illiteracy among women, or in some cases education on maternal and child health were not adequate or not available, prolong obstructed labour seem to be among the major factors associated with Vesico Vaginal fistula (vvf). Other mutilations are genital mutilation, taboos.

Statement of the Problem

Vesico vaginal fistula is one of the most debilitating health problems of women of child bearing age in our communities today. It is fast and becoming one of the most debilitating factors hindering the development of the women folk. The condition Β incapacitating the victims for the months sometimes many years, causing untold hardships to Β their families and communities at large.

This terrible health condition may lead to isolation of victim in a way that may cause social psychological consequences.

The menace of vesico vaginal fistula could be arrested or tackled if proper care is taken to understand the nature causes and consequence of the disease.

It is pathetic to observe that some women especially in rural area are left to labour for days without medical intervention, thereby creating a conducive atmosphere for the development of vesico vaginal fistula. Effort should be made to eliminate this dehumanizing health condition of women in the society since it is preventable.

Because of the physiological, social and psychological consequences of disease among the Hausa /Fulani Sokoto state, there is need to find out causes and prevalence of the disease as seen in a patient admitted in Maryam Abacha women and children hospital Sokoto.

Purpose of The Study

The main purpose of the study is to find out prevalence of Vesico Vaginal Fistula; specifically, it seeks to:

To identify the causes of Vesico Vaginal Fistula.

To ascertain the total number of women that was mostly affected from January 2015 to December 2016 in Maryam Abacha women and children Hospital Sokoto.

To determine the women age groups that are commonly affected with Vesico Vaginal Fistula

To find out the Educational status of patients as seen in Maryam Abacha Women and Children Hospital Sokoto.

To determine the preventive measures Vesico Vaginal Fistula.

Significance of The Study

The outcome of this study will be of immense benefits to the field of community Medicine, primary Health Care, Nurses, Midwifery, Women, health policy makers and community at large.

To create awareness on Birth preparedness and complication readiness, referral services and family decision making skilled attendance at every birth monitoring of every labour with the patograph for early recognition of obstructed labour ready to access to high quality emergency obstetric care, community awareness raising and education about prevention and treatment of obstetric fistula.

Early recognition of developing or developed fistula in women who have had an obstructed labour or genital trauma standard protocol at health centres for management of women who survived .Provide recommendation for policies makers on female child right act.

Scope and Delimitation of The Study

The scope of this research work will cover only the admitted patients with Vesico Vaginal Fistula Maryam Abacha Women and Children Hospital Sokoto State. Its covers from January 2015 to December 2016.The study is limited to Maryam Abacha Women and Children Hospital Sokoto due to geographical location of the research area and time factor which to complete the research was too short.
